       How do I Enroll?

 

• ​Obtain a 12-panel drug screen 

  • Where: INNOVATIVE HEALTH CLINIC  or another agency

    • Schedule an appointment on our Clinic Page​

    • If using an outside agency, that agency/facility must fax results to us-        706-850-8005 fax

• Clean Background Check​  (Click for ACC Form) 

  • ​ Must complete the form and take it to ACC Police Station Station- Follow instructions.

    • We will not accept fingerprint clearance letters​

• • Where: INNOVATIVE HEALTH CLINIC or at the

    Sheriff Department or Local Police Station (no copies)​

• Tuberculin Skin Test (PPD, TB skin test)  

  • INNOVATIVE HEALTH CLINIC (schedule appt. online) or

  • CCHD on North Harris Street​

• Complete our Application and upload the following:

• State-Issued Photo Identification​

• Social Security Card

• Copy of High school Diploma or GED  (upload in student portal)

  • (Mandated for all programs except for the CNA course
